Selective Debridement With Labral Preservation Using Narrow Indications in the Hip: Minimum 5-Year Outcomes With a Matched-Pair Labral Repair Control Group

Background
While labral repair is typically preferred over debridement, there may still be a role for selective labral debridement with preservation when applied to carefully selected cases.
Methods: This cohort study compared outcomes in 101 patients undergoing selective debridement with labral preservation (SDLP) to a matched group who underwent labral repair. Patients were followed for a minimum of 5 years.
Key Findings
The SDLP group showed significant improvements in mHHS, NAHS, and HOS-SSS scores, as well as pain reduction. Outcomes were comparable to the labral repair group across most measures, with no significant difference in return to function or satisfaction.
Conclusions
With strict indications, SDLP offers favorable long-term outcomes that may be equivalent to labral repair in carefully selected cases.
What Does This Mean for Patients
In stable labral tears, selective debridement may be a viable alternative to repair, with similar long-term results when performed by experienced surgeons.
