Outcomes of Gluteus Maximus and Tensor Fascia Lata Transfer for Primary Deficiency of the Abductors of the Hip

Background
Chronic abductor tendon tears that are irreparable can lead to debilitating gait problems and weakness. Tendon transfer techniques may offer a reconstructive option.
Methods
Case series of 3 elderly female patients undergoing gluteus maximus and TFL transfer for irreparable abductor tears. Follow-up averaged 2.1 years. Assessed gait, strength, PROs, and satisfaction.
Key Findings
- 2 of 3 patients normalized gait and improved abductor strength.
- All PROs improved for 2 patients; one showed mixed results.
- Pain scores improved substantially (VAS: 0–1).
- 2 patients were maximally satisfied with outcomes.
Conclusions
Gluteus maximus and TFL transfer may restore function and reduce pain in patients with irreparable hip abductor tears. It's a promising option in carefully selected cases.
