Outcomes of Open Versus Endoscopic Repair of Abductor Muscle Tears of the Hip: A Systematic Review

Purpose
To compare clinical outcomes and complications of open versus endoscopic repair of gluteal (abductor) muscle tears.
Methods
A systematic review of PubMed was conducted, identifying 7 relevant studies (3 open repair, 4 endoscopic repair) totaling 167 patients (127 open, 40 endoscopic). Outcomes assessed included patient-reported scores, strength, pain, satisfaction, and complications.
Key Findings
- Patient outcomes (modified Harris Hip Score, pain, strength) were similar between open and endoscopic groups at 1- and 2-year follow-ups.
- The endoscopic group often had concomitant intra-articular procedures (15/40 patients).
- The open repair group showed a higher rate of reported complications compared to the endoscopic group.
Conclusion
Open and endoscopic repairs yield comparable patient outcomes and strength improvements; however, open repairs are associated with more complications. Larger randomized studies are needed for definitive comparison.
What This Means for Patients
Patients can expect similar functional improvement with either surgical method, but endoscopic repair may have fewer complications and could be preferable when suitable.
