Purpose
To address comments made by Larson et al. regarding the authors’ previous study comparing surgical hip dislocation (SHD) and hip arthroscopy for femoroacetabular impingement (FAI).
Methods
Response letter discussing the rationale behind the study design (matched-pair cohort), addressing the learning curve, and reflecting on evolving surgical preferences. Plans for future study designs were also discussed.
Key Findings
- Matched-pair design was chosen for statistical power.
- Acknowledged the limitations of single-surgeon studies but defended their internal consistency.
- Recognized a shift from SHD to arthroscopy among surgeons.
- Committed to future broader cohort studies and encouraged multicenter collaboration.
Conclusion
Hip arthroscopy, in experienced hands, offers advantages over SHD for most cases of FAI. SHD still has a role in complex deformities.
What This Means for Patients
Patients with FAI are increasingly likely to benefit from less invasive arthroscopic surgery, though complex cases may still require open procedures. Surgeon experience is critical in achieving good outcomes.
