Birmingham Hip Resurfacing vs Total Hip Arthroplasty: A Matched-Pair Comparison of Clinical Outcomes

Background

While Birmingham Hip Resurfacing (BHR) is often preferred in younger, active patients, its clinical superiority over THA remains debated.

Methods

BHR and THA patients were matched via propensity scoring. Outcome measures included the Forgotten Joint Score-12 (FJS), VAS pain, satisfaction, and multiple PROs.

Key Findings

FJS was similar between BHR (78.0) and THA (76.0). Mental health scores (VR-12 and SF-12 MCS) were slightly higher in the BHR group. No significant differences were found in other PROs.

Conclusions

BHR and THA yield comparable short-term outcomes. While BHR may offer marginal mental health benefits, clinical results were largely equivalent.

What Does This Mean for Patients

For younger patients, BHR can be a valid alternative to THA with similar clinical outcomes. The choice should be individualized based on anatomy, activity level, and surgeon expertise.
