Endoscopic Gluteus Medius Repair With Concomitant Arthroscopy for Labral Tears: A Case Series With Minimum 5-Year Outcomes

Background

While gluteus medius repair has gained traction as a treatment for lateral hip pain, long-term outcomes with concurrent hip arthroscopy have not been well described.

Methods: Sixteen patients with partial- or full-thickness gluteus medius tears who underwent endoscopic repair and concomitant labral treatment were followed for ≥5 years. Exclusion criteria included advanced arthritis or prior hip pathology.

Key Findings

Fourteen patients (mean age 57.4) completed follow-up at a mean of 68.8 months. All patient-reported outcomes (mHHS, NAHS, HOS-SSS, VAS) improved significantly. iHOT-12 averaged 73.8, and satisfaction was high (mean 8.4/10). There were no complications or clinical repair failures; one patient (7.1%) required THA.

Conclusions

Endoscopic gluteus medius repair combined with arthroscopy demonstrates durable improvements in outcomes and pain relief at 5 years, with high patient satisfaction and low complication rates.

What Does This Mean for Patients

Patients with abductor tears and labral pathology may benefit from combined arthroscopic repair techniques, resulting in significant and lasting symptom improvement.
