Even when a labral tear is eventually identified, the next question is why it developed.

A tear may occur in association with femoroacetabular impingement, dysplasia, instability, trauma, repetitive athletic activity, or degenerative changes. Treating the labrum without understanding the mechanics of the hip may fail to address the underlying problem.

For example, a patient with hip instability may require a very different treatment strategy from a patient whose labral tear developed because of significant impingement. The labrum is part of a larger joint system, and the entire system needs to be evaluated.

When Should You Seek a Second Opinion?

Persistent symptoms deserve attention, particularly when they interfere with walking, exercise, sleep, work, or everyday activities and the initial evaluation has not provided a satisfactory explanation.

A second opinion from a physician who specializes in comprehensive hip care can provide another review of the history, physical examination, and existing imaging. In some cases, simply having the original images reviewed by a hip specialist can provide information that was not emphasized in the initial report.

The goal is not to assume that a labral tear was missed. The goal is to make sure that the diagnosis accounts for the patient's symptoms and that other potential causes of hip pain have been considered.
